My secretary says she would like to have a go at the billing, why should I consider anyone else?
There are some excellent secretaries providing a brilliant service, but unless you can verify his or her skills in this area, you are taking a chance with your money. We often hear of Consultants who chose this option, but suffer hassle when it comes to knowing what the true picture is regarding their debts, or getting stubborn debts collected.
If your secretary has come over with you from the NHS, has he or she been trained in Billing and Collection? The skills and attitude required for dealing with money are quite different to supporting your clinical needs.

How hard can it be to send out a few invoices?
It is easy to send out invoices, as long as you don’t mind the consequences.
- the recipient complains that the presentation of procedures is wrong e.g. unbundling, ineligible charges which can ultimately result in being de-registered by an insurance company.
- The way in which you calculated the charge is wrong e.g wrong calculation method for an insurance company.
- The recipient ignores the request to pay and you have no effective mechanism in place to pursue that debt.
- Other challenges are patient shortfalls, patient excesses, patient co-insurance, inability or unwillingness to pay. You see these challenges build very quickly if you do not have do not have a robust process and the time set aside to follow up on non payers.

I don't worry about non-payers. Is this a reasonable position?
Ultimately, you are in control of your own practice, but you need to be aware of the consequences of your decisions.
You would need to confirm with your Accountant on the latest position, but we understand that your tax is calculated on the Invoices that you send out, not on the amount that you collect.
This means that if you do not chase non-payers, you have already paid the tax, paid the hospital fees, paid your administration costs and you haven’t been paid.
Again our understanding is that the HMRC do not accept write-offs unless you can demonstrate that you tried to collect it.

My secretary seems to be struggling with the billing & collection, but I do not want to change things, as it may upset. What should I do?
We have taken over from, and worked with a large number of secretaries who were relieved when the billing was taken away.
